PREFINAL -2022 APRIL

  I've been given this case to solve in an attempt to understand the topic of "patient clinical data analysis" to develop my competency in reading and comprehending clinical data including history, clinical findings, investigations and come up with a diagnosis and treatment plan. Name :- Kattekola Sathwik Roll no :-58 2017 batch A28yr old tailor hailing from Urban Telangana haspresented with complaints of pain abdomen since 2 days   HISTORY OF PRESENTING ILLNESS The patient was apparently in his normal state of health 8 years ago.Then he developed pain abdomen which was sudden in onset colicky type, aggravated on taking fatty food and relieved  on medication, for which he went to a local hospital where he was evaluated and he was diagnosed with Gall stones. Then he was referred to a higher centre where MRCP was done follwed by ERCPas a therapeutic procedure. After which he was apparently normal for some months following which he developed similar episodes of pain frequent

Name :- Kattekola Sathwik Roll no :-58 2017 batch A 65 yr old farmer (man) hailing from Rural Nalgonda with a past history  of Tuberculosis  one year ago has presented  with the complaints of 1. SHORTNESS OF BREATH SINCE 4 MONTHS 2. COUGH WITH EXPECTORATION SINCE ONE MONTH 3. CHEST TIGHTNESS SINCE 15 DAYS HISTORY OF PRESENTING ILLNESS The patient was apparently  asymptomatic  one year ago. Then he developed  cough with expectoration with blood in sputum. He went to a local hospital and was diagnosed  as TB after evaluation. He took the treatment  for 5 months only . Now from about 4 months he developed  Grade 1 Shortness of breath (NYHA) which relieves on rest. There were no complaints

Then he developed high grade fever which was releived on medication but never reached base line. At the time of onset of fever, he did not have any complaints of Nausea,vomiting,cough,pain abdomen. As the fever was not releiving he was taken to the local hospital where supportive care was given . Intially she used to feel full after just eating 2 idlis. Now she skips a meal as she doesnt feel hungry  She also had complaints of distention and tightness of abdomen which started around same time which progressed to presen
